Health Concerns: In order to provide for your child’s health and safety, it is important for us to be aware of any health condition that your child may have. Please note any health problem in the appropriate space on the emergency card. This information is not only necessary for us to address each student’s health and safety, it is also necessary for any emergency personnel who may care for your student. A student’s health information is confidential and is only shared with staff on a need to know basis. If your child is injured or ill and we do not have an updated emergency card, it may be necessary to call 911 for treatment at the parent’s expense. Please include at least two or three local numbers of people who could care for your child, if we are unable to reach you.
Prior to entry into Kindergarten/First Grade, students must obtain a physical and an oral health examination.
