Dear MUSD Families,

If your child is returning to in person instruction in the hybrid model, we need you to log into the Aeries Parent Portal immediately and update your child’s medical profile. If you are not returning to the hybrid model, you can disregard this message. Once you log in, you will be prompted to update medical information. After updating your child’s medical information, please continue to the next section. In this section, it is important that you certify that your child is free of COVID-19 symptoms and that you will only bring your child to school if they show no symptoms of COVID-19 by clicking ‘Yes.’ I am attaching the full letter with daily symptom checks so that you are aware of what is required for returning to the hybrid model. You do not need to print and send in the form if you update your Aeries portal.  

You must complete the certification in Aeries Parent Portal for your child to be admitted to class. I ask that you please certify this immediately to maximize your child’s instructional time on their first day back. Please check your child’s emergency contact information and ensure that numbers and contacts are up to date.
