Appointments must be scheduled for Onsite at the below locations and times, or at a Patient Service Center (PSC)

• Bell Mountain Middle School - March 19th @ 11:00am - 3:30pm
• Menifee Union School District - March 21st @ 11:00am - 3:30pm
• Eligible to Participate:
  • Active employees and under 65 retirees enrolled on a SISC medical Plan (Anthem Blue Cross, Kaiser, WABE) and their covered dependents over age 18 are eligible for this program. 
• Appointments are required
  • New members will register and schedule online at My.QuestForHealth.com
    • Enter the Registration Key SISC2025
    • Complete the Registration Process
    • On the dashboard, members will select how they want to participate, onsite or at a PSC
  • Returning members will login and schedule online at My.QuestForHealth.com

• Colorectal Cancer Screening Kit
  • Participants (45+ years old, PPO members) will also have the option to order a kit when completing their onsite health assessment with the Quest provider.
